Job description

Key responsibilities include nursing, patient care, quality assurance, and nursing care. The purpose of this position is to provide comprehensive nursing care in a community setting, assisting patients to achieve optimal health and quality of life in situations of chronic disease. This includes providing skilled nursing services according to a physician's orders and under a plan of care. Duties include telephonic follow-up consultation, monitoring of vital signs, monitoring of blood glucose levels, preparing and administering medications, pain management, wound assessment, and providing general nursing care. The team collaborates with program managers to achieve the required service for enrolled patients
